Terms of Service
These terms are the agreement between you and Charted for using chartedbi.com, the free walkthrough, the onboarding packet, office hours, and our email and text messages. Plain language on purpose. Using the site or buying from us means you agree to them.

1. The agreement
"Charted", "we" and "us" mean [legal entity name — to be confirmed], doing business as Charted, Idaho's Independent Provider Collective, at 148 Blue Lakes Blvd N, Suite 171, Twin Falls, ID 83301. "You" means the person using the site or buying from us. Our Privacy Policy is part of these terms.
2. The free walkthrough and site content
The free walkthrough, the comparison and FAQ pages, and everything else we publish is general information based on one independent provider's experience with Idaho's own registration process. It is not legal, tax, accounting, clinical, or professional advice, and reading it doesn't create a professional relationship with us.
- We are not the state. Charted is not affiliated with, endorsed by, or speaking for the Idaho Department of Health and Welfare, Idaho Medicaid, Gainwell Technologies, or any government agency or contractor. The state's forms, instructions, and decisions control, not ours.
- The process changes. We show a last-updated date and revise the walkthrough when we learn the process has changed, but you are responsible for confirming current requirements with the state before you rely on anything here.
- No promise of approval. Nothing on this site guarantees that the state will approve your enrollment, that a background check will clear, or that any application will be accepted.
3. Who can use Charted
You must be at least 18 and using Charted for your own professional practice or business. You agree to give us accurate information and to keep it current.
4. The onboarding packet
When we offer the paid onboarding packet, these terms apply to it:
- Price and payment. The price is the one shown at checkout at the time you buy. Payment is processed by Stripe; by paying you also agree to Stripe's terms. Prices can change over time, but a price you've already paid is final.
- What you get. A personalized packet generated from the information you enter at purchase, delivered by email as a download link, plus access to office hours for your cohort and to the community. You will be asked to accept these terms at checkout, and we record that acceptance with your purchase.
- Re-downloads. Your packet stays available to re-download using the email address you paid with. Each re-download serves the same saved file; a new file is only generated through the update process in section 6.
- Watermarking. Every packet carries your name and purchase details as a visible watermark and as embedded document metadata. That's how we tell a legitimate copy from a redistributed one.
- Your information must be accurate. The packet is only as correct as what you enter. Review it before you use it; you are responsible for what you submit to the state.
5. Your license to the packet
When you buy the packet we grant you a personal, non-exclusive, non-transferable license to use the documents in it for your own independent-provider practice. You may print it, fill it in, and submit the completed forms to the state, physicians, and caseworkers as part of your own work.
You may not:
- sell, rent, share, post, or otherwise redistribute the packet or any part of it;
- repackage it, or material derived from it, into your own course, product, template, or service;
- remove or alter the watermark or metadata;
- use it to help someone other than yourself set up their practice, other than by telling them about Charted.
Charted and its author keep all ownership of the packet's content. If you break this section, your license ends immediately and we may pursue the remedies the law provides, including for copyright infringement.
6. Updates and office hours
- Update requests. If something on your packet needs to change — a new address, a corrected identifier — you can ask us to regenerate it. Each purchase includes up to [number — proposed: 2] free update requests; beyond that, updates are handled by arrangement. Changes to identity fields (your name, business name, or provider identifiers) are reviewed by a person before a new packet is issued, because those fields are what the watermark is built from.
- Office hours. Live office hours are run per cohort with a capped number of seats. They're held on the schedule we announce; we'll give reasonable notice if a session moves. Office hours are for questions about the process; they are not legal or clinical advice, and the "no promise of approval" rule above applies.
7. Our guarantee and refunds
We can't guarantee state approval — but we can guarantee the process. Follow the packet and attend office hours. If your application still bounces for a paperwork error, we fix it and help you resubmit — free.
To be clear about the edges: the guarantee covers paperwork errors in material the packet produced. It doesn't cover denials for reasons outside the paperwork (eligibility, background-check results, licensing, state policy), information you entered incorrectly, or applications submitted without following the packet's steps.
Refunds. [Decision needed. Proposed: because the packet is personalized to you and delivered digitally, purchases are not refundable once your packet has been generated, except where the law requires otherwise. If you have a problem with your purchase, email us — we would rather fix it than argue about it.]
8. Text messages (SMS Terms)
These SMS Terms apply if you opt in to receive text messages from Charted.
Program: Charted Updates.
Opting in: you opt in by providing your mobile number and checking the SMS consent box on a form at chartedbi.com. Consent to receive texts is not a condition of receiving the free walkthrough or of purchasing anything.
Messages you'll receive: marketing and promotional text messages from Charted, such as updates about the free walkthrough, the onboarding packet, office hours, and the community. Message frequency varies.
Opting out: reply STOP to any message at any time. You'll receive a single confirmation message and no further texts unless you opt in again. You can also email hello@chartedbi.com and we'll remove you.

Message and data rates may apply. Your carrier's standard messaging and data rates apply to texts you send and receive. Check with your carrier for details.
Carrier disclaimer: Charted and participating carriers are not liable for delayed or undelivered messages. Delivery depends on your carrier and other factors outside our control.
Privacy: see section 5 and section 6 of our Privacy Policy. Your mobile information is never shared with third parties or affiliates for their marketing.
You confirm that the mobile number you give us is yours, that you're the account holder or have the account holder's permission, and that you'll tell us if the number changes hands.
9. Email
Signing up for the free walkthrough means we'll email it to you and, afterwards, send occasional updates about it and about Charted. Every marketing email has an unsubscribe link. Transactional email — receipts, download links, replies to your questions — is sent as part of the service and isn't affected by unsubscribing from marketing.
10. Acceptable use
Don't use the site or our forms to send spam, submit false or someone else's information, probe or overload our systems, scrape content, or do anything unlawful. We may block or remove anyone who does.
11. Ownership
The site, its text, design, the Charted name and logo, the walkthrough, and the packet are owned by Charted or its author and protected by copyright and trademark law. You may share links to our pages freely. You may not copy, republish, or build on our content beyond what section 5 allows without written permission.
12. Third-party services
We rely on other companies to run parts of Charted — Stripe for payments, Cloudflare for hosting and bot protection, Resend and GoHighLevel for email and text delivery, and Skool for the community. Their services are governed by their own terms, and we aren't responsible for them. Links to state websites and other third parties are provided for convenience; we don't control those sites.
13. Disclaimers
The site, the walkthrough, the packet, and office hours are provided "as is" and "as available." To the fullest extent the law allows, we disclaim all warranties, express or implied, including fitness for a particular purpose and non-infringement. We don't warrant that the site will be uninterrupted or error-free, that the content is complete or current at all times, or that any outcome — including enrollment, approval, or income — will result from using it.
14. Limits on liability
To the fullest extent the law allows, Charted and the people behind it won't be liable for indirect, incidental, special, consequential, or punitive damages, or for lost profits, lost revenue, lost clients, or lost data, arising from the site or our services — even if we were told they were possible. Our total liability for any claim relating to Charted is limited to the amount you paid us in the twelve months before the claim arose, or $100 if you've paid nothing. Some jurisdictions don't allow some of these limits, so some may not apply to you.
15. Indemnification
You agree to defend and hold Charted harmless from claims, losses, and expenses (including reasonable attorney's fees) arising from your breach of these terms, your misuse of the packet, or information you submitted to the state or anyone else.
16. Governing law and disputes
These terms are governed by the laws of the State of Idaho, without regard to conflict-of-law rules. If you have a problem, email us first — most things get sorted out in a conversation. If we can't resolve it informally, both sides agree to try mediation before filing suit. Any lawsuit will be brought only in the state or federal courts located in [Twin Falls County — confirm], Idaho, and you consent to their jurisdiction. To the extent the law allows, both sides waive a jury trial.
17. Changes
We may update these terms. When we do, we'll change the date at the top; for material changes affecting an existing purchase or an active subscription to our messages, we'll email you first. Continuing to use Charted after a change means you accept it.
18. The rest
If part of these terms is found unenforceable, the rest still applies. Our not enforcing a term isn't a waiver of it. You can't transfer this agreement to someone else; we may transfer it as part of a sale of the business. These terms and the Privacy Policy are the whole agreement between us about Charted.
